Tdap Vaccine protection against tetanus, diphtheria, and pertussis

What is the Tdap Vaccine?

The Tdap vaccine offers combined protection against tetanus, diphtheria, and pertussis. It is administered via intramuscular injection, providing essential immunity, especially for travelers, healthcare workers, and individuals exposed to higher risks.

What is Tetanus?

Tetanus, often called "lockjaw," is a serious bacterial infection caused by Clostridium tetani. The bacteria enter the body through open wounds, releasing toxins that affect the nervous system. Symptoms include muscle stiffness, jaw cramping, muscle spasms, difficulty swallowing, and painful contractions. Without treatment, tetanus can lead to complications such as respiratory failure or even death.

What is Diphtheria?

Diphtheria is a bacterial infection caused by Corynebacterium diphtheriae, primarily affecting the nose, throat, and occasionally the skin. Symptoms include a sore throat, fever, and swollen glands, with a thick gray membrane forming in the throat, potentially blocking the airways. Diphtheria can also lead to heart and nerve damage if left untreated.

What is Pertussis?

Pertussis, commonly known as whooping cough, is a contagious respiratory disease caused by Bordetella pertussis. It begins with symptoms similar to a common cold, progressing to severe coughing fits that can lead to vomiting or a distinctive “whooping” sound. In Thailand, pertussis cases have been rising, with three recorded deaths in 2023. Pertussis can be mild in adults, but they can still transmit it to unimmunized children, posing a serious health risk.

Who Needs the Tdap Vaccine?

The Tdap vaccine is recommended as follows:

  • Adults: A booster dose every 10 years to maintain immunity.

  • Pregnant Women: To pass immunity against pertussis to the newborn, the Tdap vaccine is recommended during each pregnancy, starting at 20 weeks gestation.

  • Individuals with Certain Wounds: The Tdap vaccine is also recommended for wounds with high risk of infection, such as:

    • Dirty wounds untreated for more than 6 hours

    • Puncture wounds

    • Necrotic tissue wounds

    • Contaminated or sepsis-associated wounds

    • Burns, frostbite, or high-velocity injuries

    • Animal bite

How Many Doses of the Tetanus Vaccine Are Needed?

For effective immunity, it’s recommended to receive a single-dose booster immunization with Tdap every 10 years. If you are unsure of your vaccination history, an initial series of three doses at 0, 1 month, and 7 months is recommended, with Tdap followed by dT (tetanus and diphtheria) to ensure lasting immunity.

Did you know that cases of whooping cough, or pertussis in Thailand, have been increasing? In the year 2023, there were approximately three deaths from this disease, and further studies in Thailand have found that 18.4% of adults experiencing chronic cough for at least two weeks were diagnosed with pertussis. In adults, symptoms of whooping cough may be mild, but what is concerning is that adults can spread this disease to children who are not yet immunized, leading to illness or even death.

Hence, it is recommended to receive tetanus and diphtheria (dT) or

tetanus, diphtheria, and pertussis (Tdap) vaccinations every 10 years to boost immunity and reduce the incidence of these diseases in adults.
